At UAP Clinic, we know our patients live busy lives. That's why we are pleased to offer online services to help you refill your prescription, order contact lenses or ask questions about your bill with a simple click of a button.

New Online Service Available -
Request an Appointment Online

Did you know that our online services now include an appointment request page?

If you don't want to phone in for your next appointment, you can access the "Request An Appointment" page from the Online Services drop-down menu. Log in, fill in the information on the form and hit "Send." Someone from the Clinic staff will contact you, normally within one business day, to set up your appointment. (Please note, this should not be used in case of an emergency; if you are having an emergency, please call 911.)

The form is secure and information remains confidential. If you haven't set up a My UAP Clinic account, you can do so on your first visit to the page.
