Patient & Visitor Center
At AP&S Clinic, we are working every day to become the preferred provider of primary and specialty care to residents of western Indiana and eastern Illinois. One of the ways we are working toward this goal is by evaluating current and implementing new systems to make your time with us as simple as possible.

At AP&S Clinic, we utilize a centralized check-in process for registration. We will verify information, copy your insurance card, and have you sign an authorization allowing us to bill your insurance and receive payment.

Please bring a complete list of your current medications and your insurance card to each office visit. We will verify your demographic and insurance information each time you come in for services. Accurate information allows claims to be processed efficiently and payments from insurance companies to be made in a timely manner.

Appointments are scheduled by office staff at each location. Hours are 8 a.m. to 5 p.m., Monday through Friday with services on Saturday mornings at AP&S Northside Family Medicine and the AP&S Optical Shop. You may reach our offices 24 hours a day by calling the numbers listed in our phone directory. After hours, our calls are forwarded to our answering service.
